The date on which this Certificate of Service is being prepared and issued.

State the employee's last held job title or provide a brief, accurate description of the work they performed.

The exact date the employee first commenced employment with this employer.

The employee's last official day of employment (termination date).

State the employee's rate of pay as at the date of termination. Specify the amount and frequency (e.g., 'R30,000 per month', 'R180 per hour', 'R7,000 per week').

If the employee was paid out for their notice period instead of working it, state the amount paid and the period it represents (e.g., 'R30,000 (1 month's notice)'). If notice was worked or not applicable/waived, enter 'N/A' or 'None'.

Only applicable in specific circumstances, primarily retrenchment/operational requirements dismissals. If severance pay was paid according to the BCEA (min. 1 week per completed year of service) or an agreement, state the amount paid. Otherwise, enter 'N/A' or 'None'.

CRITICAL: The BCEA states the reason for termination should ONLY be included if the *employee requests it*. The default should be 'No'. Only select 'Yes' if you have a written request from the employee.
